YouthMembershipApplication.pdf Use this Youth Package Application to open a new account for a minor, which is accessible by the minor. This account requires an adult co-applicant. This form also includes a request to open a Youth Add-On Certificate, Youth Checking account, and to make changes on the account.
Checking Application.pdf Use this Checking Account Application [PDF] to just open a new Youth Checking account, for a youth who is already a member. Remember to list the adult as co-applicant.
Deposit Application.pdf Use this Deposit Application [pdf] to just open a new Youth Certificate, for a youth who is already a member. Remember to list the adult as co-applicant.
Transfer Funds Request.pdf You can use this Transfer of Funds Request to fund the new account(s) by transferring funds from your account at another institution.
Transfer Funds Request.pdf After the youth account is open, you can easily transfer funds from your account to the child's from within Account Manager, if you set up Cross Account Transfers this with form.

These forms can be completed either before or after printing them. Sign your form(s) and mail with your initial deposit to:

These applications require the Adobe Acrobat Reader plug-in, which is available free from Adobe. Or, you may also receive an application by stopping by or calling one of our branches, or sending us an e-mail requesting that we mail you an application.

These accounts cannot be opened without an initial deposit (minimum $5). You may use a
check or
money order,
or transfer
funds either
from an existing Provident Credit Union account
(enter the co-applicant's specific account number on the application) or
from another institution
(include a completed Transfer of Funds Request).

Identification Required

Please include a photocopy of a Driver's License or State I.D. for the adult signer.
A Master Account Disclosure and Service Charge Schedule will be sent to you upon approval of your membership application.
